remove semi-functional ElementFormattingStage

Description

The existing ElementFormattingStage code doesn't work very well, and is also completely dependent on Xalan being the XSLT implementation.

Until we have a chance to address https://shibboleth.atlassian.net/browse/MDA-64#icft=MDA-64 properly, we should remove the current ElementFormattingStage. It's not documented at present, so this should just be a question of deleting it.
